Slow breathing produces changes you can measure on instruments, not just changes you report feeling. That distinction matters in a field full of claims that rest entirely on how people say they feel afterwards.
What does the study actually show?
In a controlled study published via PubMed Central, participants breathed at a slow pace of six breaths per minute and were compared against a normal rate of twelve.
Two findings stand out. First, baroreflex sensitivity, the reflex that adjusts your heart rate in response to blood pressure changes, improved: "BRS increased from pre to post slow breathing by 10% (p = 0.012)." Second, a standard heart rate variability measure "increased during the slow breathing condition only and remained elevated above baseline during post-testing."
Read that second quote carefully, because it contains the practically useful part. The effect appeared only in the slow condition, so it was not simply the result of sitting quietly. And it persisted after the exercise finished rather than stopping when the breathing did.
What is heart rate variability, in plain terms?
It is the variation in time between consecutive heartbeats. A healthy heart does not tick like a metronome; the gaps flex slightly as your autonomic nervous system adjusts. Greater variability is generally taken as a sign that the system is responsive rather than locked in a fixed state.
That is why it turns up so often in breathwork research. It is one of the few things in this area that can be measured objectively from outside the person.
Does this hold up across the wider literature?
Reasonably well. A systematic review of 58 clinical studies, comprising 41 randomised controlled trials and 17 single-arm or non-randomised trials, reported that "breathing practices significantly reduced stress/anxiety in 54 interventions".
That is a consistent direction of effect across a substantial body of work. It is not the same as saying breathwork treats anxiety as a clinical condition, and the studies vary a great deal in technique, duration and population. The fair summary is that deliberately slowing your breathing reliably makes people feel less stressed in the short term, which is unusually well supported for something that costs nothing.
Six breaths a minute sounds slow. Is it?
It is about half a typical resting rate, and it works out to roughly five seconds in and five seconds out. Most people find it comfortable within a minute or two, though it feels deliberate at first rather than automatic.

How to actually do it
- Sit upright somewhere you will not be interrupted. Standing is fine too; lying down tends to blur into dozing.
- Breathe in through the nose for about five seconds. Comfortably full, not maximally.
- Breathe out for about five seconds. Slower than the inhale is fine and often easier.
- Keep it unforced. If you are straining, shorten the count rather than pushing through.
- Continue for five minutes. That is the dose used in the trial with the strongest daily mood effect.
A caution worth keeping
Slow breathing is gentle by definition, and NCCIH considers relaxation techniques generally safe for healthy people, while noting rare reports that they might cause or worsen symptoms in people with epilepsy or certain psychiatric conditions, or with a history of abuse or trauma.
