The most important thing to understand about herbal remedies is not which one works. It is that supplements are not reviewed for safety or effectiveness before they reach the shelf, and that several popular herbs interact meaningfully with prescription medicines. Once you know those two things, you can read the category properly. The evidence for the best-known adaptogens is genuinely thin, and the risks, while modest, are real and specific.
What follows is how the regulation actually works, what the National Center for Complementary and Integrative Health says about the most popular herbs, why interactions deserve more attention than efficacy, and what to ask before taking anything.
How are supplements actually regulated?
Differently from medicines, and much more loosely than most people assume. NCCIH states it plainly: "In all other cases, the FDA is not authorized to review dietary supplements for safety and effectiveness before they are marketed." The FDA says the same of itself: it "does not have the authority to approve dietary supplements before they are marketed."
So a product on a shelf has not passed a pre-market check for whether it works or whether it is safe. That is not a conspiracy, it is how the category is legally defined. But it does mean the burden of scepticism sits with you in a way it does not for prescription medicine.
Does natural mean safe?
No, and NCCIH addresses this directly with a concrete example: "Many dietary supplements (and some prescription drugs) come from natural sources, but 'natural' does not always mean 'safe.' For example, the kava plant is a member of the pepper family but taking kava supplements can cause liver disease."
The liver comes up repeatedly in this field, and not by coincidence. It is the organ that processes most of what you swallow, which makes it the first place concentrated plant extracts tend to show up as a problem.
What does the evidence say about the popular ones?
Less than the packaging implies. Here is what NCCIH actually concludes about the three most asked-about.

Read the turmeric line again, because it is the one people find most surprising. This is a federal research centre saying it cannot definitively conclude a benefit for any health purpose, about one of the most widely sold supplements in the world. That is not the same as saying it does nothing. It means the evidence has not established that it does something.

Why do interactions matter more than efficacy?
Because a herb that does nothing is a waste of money, while a herb that alters your medication is a medical problem. St John's wort is the clearest case: NCCIH lists interactions with amitriptyline, bupropion, birth control pills, cyclosporine, phenytoin, carbamazepine, digoxin, ivabradine, indinavir, nevirapine, irinotecan, imatinib, docetaxel, warfarin and simvastatin.
That list spans antidepressants, contraception, transplant medication, anticoagulants, HIV medication and chemotherapy drugs. Reduced effectiveness in any of those is a serious outcome.
It is not only St John's wort. NCCIH's guidance on how herbs interact with medicines notes that "Concentrated green tea supplements can reduce blood levels of some drugs, such as the cholesterol-lowering drug atorvastatin and the beta-blocker nadolol." Green tea is about as innocuous as a supplement sounds, which is precisely the point.
What is the single most useful habit?
Disclosure. NCCIH's advice is straightforward: "When you visit your health care providers, it's important to tell them about all the medicines and supplements you take."
People routinely omit supplements when asked what medication they are on, because a supplement does not feel like a drug. From an interaction standpoint it can behave like one. Keeping a list on your phone and showing it at appointments is a two-minute habit that closes most of the risk in this entire category.
How do you read a supplement label sensibly?
- Assume nothing has been pre-approved. Neither the effectiveness claim nor the safety of the formulation has been reviewed before sale.
- Check what the actual ingredient is. Extract type and concentration vary enormously between products with the same headline name.
- Treat "high absorption" as a flag, not a feature. NCCIH specifically notes that highly bioavailable curcumin formulations may harm the liver.
- Look up interactions before you buy, not after. Particularly if you take any prescription medication.
- Be sceptical of proprietary blends. If you cannot tell how much of what you are taking, neither can a clinician trying to help you.
- Tell your provider. Add it to the list you actually share.
Who should be especially careful?
- Anyone taking prescription medication, given the range of documented interactions.
- Anyone with liver disease, or taking anything else that stresses the liver.
- Anyone who is pregnant or breastfeeding.
- Anyone scheduled for surgery, since some supplements affect bleeding and anaesthesia.
- Anyone giving supplements to a child.
In each of those cases the question is not which brand, it is whether at all, and that is a question for a clinician or pharmacist.
What this guide will not tell you
We will not give you a dose. We will not tell you which brand to buy, or that a particular herb will fix a particular symptom. Doses in this category vary by preparation and by person, the evidence for most of these herbs does not support specific claims, and the interactions are genuinely consequential. Anywhere you see a confident dosage recommendation for an adaptogen alongside a purchase link, treat the two as related.
